Acquisition, Selection and Intake: Getting More Patients for Your Hospital at Home Program : The task of recruiting patients into a Hospital at Home program is generally called intake. Intake can be much more challenging than envisioned in the development phase of H@H programs prior to implementation. "Out of 1000 beds there must be at least 20-30 patients daily that meet inclusion criteria for our program" is a comment this speaker has heard more than once. Successful intake requires a robust and timely screening process, balanced service-line outreach and collaboration, creativity, institutional awareness and alliance, operations that maximize intake opportunities, and a team that is flexible and goal-oriented. Getting all of this right daily can be challenging but gets better over time. This session will explore these issues and more.
