Michael Chin, MD, Associate Medical Director, MassHealth and Assistant Professor, University of Massachusetts Chan Medical School

Michael Chin is an Assistant Professor of Family Medicine & Community Health at the University of Massachusetts Chan Medical School, and since July 2021 has been an Associate Medical Director at MassHealth (the Medicaid program for the state of Massachusetts). Michael’s areas of focus at MassHealth have included health equity, health information technology and clinical informatics. Michael is a board certified physician in family medicine, and his clinical work has included over 10 years as a primary care and urgent care physician at two federally qualified community health centers in Massachusetts.
Prior to joining MassHealth, from 2006 to 2013, Michael was a Senior Policy Analyst for the Massachusetts Health Insurance Connector, which was the nation’s first state-sponsored health insurance exchange. From 2016 to 2108, Michael was a Policy Analyst supporting the Mass HIway, which is the Massachusetts state-sponsored health information exchange."

GENERAL SESSION :
Health Equity and Hospital at Home: Hospital at home or home-based acute care is in the position to bridge the gap of health equity. The innovative care-delivery model is helping to better assess patients’ social determinants of health through access to patients’ home environments. The session will discuss some of these important benefits and explore existing data on hospital at home and health equity. The panelists will also explore aspects that providers should consider related to social and safety screenings; potential impacts on caregiver burden; language, social and culture care congruencies; and the continuum of post-acute care. Join this panel session to explore these important topics and more.
