Brooke Miller, PharmD, BCACP, Team Lead, Hospital-Level Care at Home, Intermountain Health
Brooke Miller, PharmD, BCACP is a clinical pharmacist at Intermountain Health and Hospice in South Jordan, Utah. She serves as a team lead overseeing operations of Hospital-Level Care at Home and the pharmacy intern program.
Dr. Miller graduated with her PharmD from Roseman University of Health Sciences (Utah). She completed a PGY-1 residency in Community-Based Care and maintains board certification in ambulatory care pharmacy. She is currently pursuing her Master of Health Care Administration (MHA) degree from Western Governors University.
Areas of practice experience include inpatient acute care, ambulatory care, anticoagulation, diabetes management, community pharmacy, pharmacy leadership, and infusion pharmacy services.
Dr. Miller is actively involved with the growth and advocacy of Hospital at Home and has presented education sessions and posters for American Society of Health-System Pharmacists (ASHP), Hospital at Home User’s Group, National Home Infusion Association (NHIA), Hospital-at-Home Leadership Summit, and American Academy of Home Care Medicine (AAHCM).

Working Group Discussion: Pharmacy Services to Support Hospital at Home
This working group will explore the complex issues around pharmacy and hospital at home, including: home medications, opiates, limited formulary, symptomatic management, documentation and supervision, and transition to discharge. Participants will have the opportunity to share experiences, address common issues, and find solutions among industry peers.
